What do you need to know?
The administration of opioid agonist maintenance therapy (using methadone, buprenorphine and others) by medical professionals not integrated in accredited centres requires an authorisation from the Ministry of Health.
This procedure can only be carried out online. Documentation provided through any other channel will not be taken into account or counted for the purposes of the submission date.
Specific forms have been made available for the applications and communications relating to this procedure, which must be used. They can be downloaded from this page, and you should always use the latest published version. Applications and communications submitted by any means other than these forms will be deemed not to have been submitted.
- Medical professionals not integrated in accredited centres.
These professionals are obliged to interact with the public administration by electronic means to carry out any administrative procedure according to Article 14.2 of Law 39/2015, of 1 October (Official State Bulletin (BOE) no.236, of 2 October 2015) and Order PDA/20/2019, of 14 February (Bulletin of the Government of Catalonia (DOGC) no. 7814, of 20 February).
Doctors not integrated in accredited centres must inform the Ministry of Health when they cease to provide treatment with opioid agonists.
No fees are associated with this procedure.

This procedure can only be carried out online. If applying on behalf of a legal entity, the form must be signed using a digital certificate of representation.
If applying in your own name, you may sign using idCAT mòbil or a digital certificate.

You can fill in the form online without having to download it.
- Fill in the form and attach the requested documentation. To attach the documents, click on the Attach button and search for the appropriate document on your computer.
- The name of the attached file must not contain blank spaces, hyphens, accents or diaeresis.
- Send the form. If mandatory information is missing or entered incorrectly, an error message will be displayed when you submit the form and the fields to be revised before resubmitting it will be highlighted.
- The form must be signed with a digital certificate of representation or the digital certificate of the legal entity. See How to sign an HTML form digitally.

Confirmation of the procedure carried out
If the application has been sent correctly, an acknowledgement of receipt will appear on the screen (you will need Adobe Acrobat to open it). It's important to save this document as it contains the entry record, which is the date the procedure was initiated and may be relevant for specific calculations and deadlines. It also contains an identifier code for the procedure, which you can use to check the status of the procedure from the Private area.
If the application contains any errors, it will not be sent, and a message will be displayed specifying the error.
If you are unable to complete the procedure due to a glitch in the system, please try again later.
You can check for any service interruptions or incidents in the Electronic office service interruptions and notifications section, which contains information on the types of incidents and where they occurred. Search for service incidents for the appropriate year and sort by Start date and time, in descending order.

Status of my transactions
- Go to Status of my transactions.
- Enter the alphanumeric transaction identification code (ID) provided on the acknowledgement of receipt of the transaction and the identity number (DNI, NIE, passport, company NIF or foreign company document) that was entered on the application form. This identification code allows you to check the status of your transactions without a digital certificate.
You can obtain information about the current status of a procedure or transaction from the relevant procedure details page.
Private Area
Keep track of all your procedures and transactions and update your personal details.
To access the Private Area, you must identify yourself using a digital certificate or a one-time password.
